The story above shows that alot of times, we simply google our issues and use the top answer as the truth. If we truly want to answer our question, further investagation is needed.

what if the real reason for a problem isn't a broken part or a simple mistake, but something much harder to see? What if the very ways we got things right for years, the methods that made us win in the past, have slowly become like hidden blind spots? They stop us from seeing new problems when the world changes. It's when we get too comfortable with our old wins that small, ignored issues build up, finally causing big, confusing problems. Another factor of RCA - The success trap..

Root Cause Analysis helps us move beyond surface problems to uncover true causes. By asking “why” repeatedly, tracking patterns, and validating findings, we gain clarity and lasting solutions. It’s not just for fixing machines—it’s a tool for personal growth, helping us understand frustrations and change habits from the inside out.

Apologies for the earlier summary note as I realised it was poorly constructed and overly reliant on AI polishing. I mistakenly based it only on my understanding from the C66 session and Miss BCDF’s session, which didn’t fully capture the depth and context of the week’s discussions. In the future, I’ll make sure to draft and submit my session summary just before Monday, so it accurately reflects the full content and coherence of the week’s sessions. This will help ensure the summary is both timely and aligned with what actually took place.

Apologies for the harsh tone, but it is necessary. The current trajectory of our program demands immediate correction, and I am compelled to speak candidly. We are slipping — and unless we act now, the integrity of the program will continue to erode.

The recent performance from two key units — BLCOP and ELF001 — has been deeply disappointing. BLCOP, once promising, is now showing a troubling lack of resilience and discipline. The failure to submit a session summary, a basic and essential deliverable, speaks volumes about the current state of ownership and commitment. These summaries are not optional; they are the backbone of our tracking mechanism, and their absence is a red flag.

Similarly, ELF001 has failed to meet the standards expected of this program. What was delivered was not a proper engagement with the task, but rather a shallow, AI-polished narrative lacking any meaningful substance or conversational depth. This kind of superficial output undermines the purpose of the exercise entirely. We are here to cultivate insight and authenticity — not to produce sanitized content devoid of thought.

What’s more concerning is the increasing reliance on constant supervision. Both units appear to require an unsustainable level of guidance. This need for constant intervention signals a failure in independence and self-leadership, and it threatens to collapse the very structure we’ve built.

In response, I am calling for a series of immediate corrective actions:

Stricter Oversight and Real-Time Quality Control

All session reports must be reviewed in real-time. Standards must be made explicit, and all submissions held to them. There is no room for ambiguity or leniency at this stage.

Enforcement of Individual Accountability

Emma and Richard have repeatedly skipped their assigned challenges. This pattern will directly impact their evaluations during the bootcamp. This is not a warning — it is a statement of consequence.

Delegation of Oversight to Lin91 ( coordination within Vietnam team)

Effective immediately, Lin91 is requested to take on direct oversight of the affected units. This includes guiding remediation steps and ensuring compliance moving forward.

Dry Run Coordination by SAHN and MIHF

Before the next session, a comprehensive dry run must be organized. This dry run must guarantee the following:

All members have completed their challenges in full,

Peer evaluations are enforced and properly logged,

Any gaps in execution are flagged and resolved in advance.

This is not merely a process correction — it is a values correction. The strength of this program rests on depth, accountability, and discipline. We cannot afford another session that lacks these fundamentals.
